If you are a Connecticut State employee and you wish to apply for a disability retirement, your agency may be handing you a new form. It is a form which states, “I wish to waive my right to be present at my disability retirement hearing.”  Think carefully before you sign this form! If you have an attorney for your state disability retirement case, ask your attorney about this. The same goes for any other state form you are given during the process of applying for a disability retirement.  Don’t sign until you have discussed the form with your attorney.
