The Journal Gazette – The Department of Veterans Affairs said Tuesday it is investigating how a Fort Wayne military veteran mistakenly received medical records intended for two other veterans. “We’re going through a thorough review this week of our patient privacy program,” Jay Miller, acting director of VA Northern Indiana Health Care System, said in a phone interview.
